The problem
Postpartum and PACU nurses at hospitals after C-sections face a recurring bedside wiring problem where blood tubing is left as the primary line and medications like oxytocin are hung onto the wrong set. The dangerous moment is during OR→PACU handoff when a med is clamped to blood tubing, causing possible uterine atony, transfusion, or a rapid response with costs of thousands per event and lost clinical hours. Existing EMR rules, stickers, and classroom training all miss the physical bedside mismatch and do not block a wrong mechanical connection.
